When setting a “Monthly Event” that is “Repeated on” a particular day of the week, “Fifth” is not present in the drop down.

Fifth is not always possible, there is the option Last that will handle that case

Exactly, but the "last" option DOES NOT handle that case. I have client that specifically has an event every few months because it is the "fifth". Using "last" will result in the event being scheduled every month will it not?

Using Last is indeed EVERY month last. 

Fifth (use only for months that have a fifth) is not an option.
