At some point we acquired this malformed HTML at the top of all of our pages.  

 

The HTML actually shows the text of this: 

<html>
xml:lang="en-US" lang="en-US" xmlns="http://www.w3.org/1999/xhtml"

It's across all themes in our installation and all portals.  Can't find anything in the skins folder or default.aspx.  

 

Does anyone know where else this could be coming from?

It looks like there are no opening and closing tags on the xml code.

Has anyone changed the skin lately?
Or, I wonder, because I don't generally touch this is there a setting in the admin or host section that allows you to specify fallbacks?

It's all skins, so it's above that level.  We aren't sure when the change occurred.  Going back through source control and can't seem to see anything that should affect display on the entire site.
