The form and list module has been my goto solution for a long time.  However, now whenever I use the latest version it consistently doesn't work any more for me.  Everything acts like it sends, confirmation message, saves to db, but it doesn't arrive via email.  I know my smtp setup is working from other modules on the same installs, and emails are sent along with my smtp tests. 

If I review the site log it shows an error for that module and on that page (red) and gives a message of  "Mailbox unavailable. The server response was: Sender is not allowed." 

Any suggestions on how to get this thing working again? I have numerous sites now not working.

it looks like you specified wrong sender email address, maybe log of SMTP server will provide more details on it

hmmm.  In the past I have never configured the module to use the same smtp email address and the sender email address.  They have always been different. 

I configure the smtp host settings, test it, and then configure the Form/List module with a sender email address and to email address.  Are you suggesting these need to be the same now? 

I also use the feedback module in much the same way and on all of these installs that module (feedback) sends just fine.
