Sorry I'm not clear whether you are trying to pass a parameter into he feedback module, or pass one out. What parameter are you trying to pass to where?

Sorry, maybe I am wording it wrong.  I want my user to be able to report to me the URL and title of the video in question.  Is there a way to create a feedback field that automatically gets the URL and video title?

Not with the current module by default, though I imagine you could write some JavaScript to populate the message field of a feedback module instance placed on your video page.
